Sophia and Grace-Anna are addressing chronic disease progression, functional fitness, and quality of life among adults of all ages in the underserved West End of Birmingham, Alabama, through providing a sustainable, community-based fitness intervention in partnership with Cahaba Medical Care. Ultimately, the program will aim to not only decrease sedentary behavior and improve cardiovascular/muscular fitness in participants, but also to provide a space for Cahaba staff to care for and foster community with those they serve in the West End. They will also aim to enhance participant knowledge and confidence in managing their health through regular movement and peer support as well as create a replicable and culturally tailored program model that can be sustained by community health workers and clinic staff beyond the funding period.
